Our goal is to provide you the quality care that you deserve. We do our best to streamline the process and be respectful of your time. You can help us out by:

  • Downloading and completing a new patient history/intake form

  • Try to arrive 30 minutes prior to appointment time if possible to aid in completion of any remaining paperwork and to allow time for any additional x-rays Dr. Corpus may require prior to your visit

  • Bring your current ID and valid insurance card

  • Bring a referral authorization from your primary care provider, if your insurance requires

  • Bring a medical history to include all current medications and dosage

  • Bring all previous x-rays, CAT scans, MRI scans, or other test results relating to your medical problem (both discs with images as well as radiology reads if available)

  • Bring previous medical records relating to current medical problem (including operative reports if any previous surgery has been performed)

  • List of questions for Dr. Corpus

It is very important to us that you understand your injury and proposed treatment. It is normal to feel somewhat overwhelmed after a visit and therefore very helpful and important to review the educational material compiled by Dr. Corpus under the Patient Education section of this website.

You are encouraged to ask questions throughout the evaluation and treatment process. A written list of questions can be quite helpful. If questions remain after your visit, please feel free to call the office or email the practice at peoriasportsdoc@gmail.com.

If a test or study is ordered as a result of your visit, Dr. Corpus can either see you back for a visit and go over the images in person, or call you with the results to further develop a treatment plan depending on your schedule and availability.
